DESCRIPTION OF OCCURRENCE:
On 05/24/16 male patient (A), in his 20s, was punched by male patient (B), who was in his 30s.
The facility conducted an internal investigation and notified the police. Both patients had mental health diagnoses. Staff witnessed patient (B) lunge at patient (A), punched patient (A) repeatedly, and pinned him against the nurse's station. Patient (B) was shouting that patient (A) was looking at him funny. Staff intervened and placed patient (B) in a one-person progressive restraint technique. Patient (B) remained combative, tried to get away, and began striking the staff member. Patient (B) was placed in seclusion and restraints. Patient (A) was assessed and noted to have an area of redness on his upper left torso and left jaw. Patient (A) denied the need for treatment. The following morning, resident (A) was noted to have bruising on his chest, but continued to refuse treatment. Patient (A) denied being fearful of resident (B). The incident was witnessed, thus, the allegation was substantiated. Patient (B) was placed on assault II precautions and moved to a different wing on the unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
