DESCRIPTION OF OCCURRENCE:
On 05/06/16 female patient (A), in her 40's, informed staff that female patient (B), in her 30's, punched her in the face twice.
The facility conducted an internal investigation and notified the physician and police. Patient (A) approached staff and stated patient (B) punched her in the face twice when she was in the shower room. Patient (A) alleged she entered the shower room and patient (B) yelled, "Get out of here!" to which patient (A) replied, "I have a right to be in here." Patient (B) allegedly punched patient (A) directly in the face two times and walked out of the shower room. Staff intervened and escorted patient (B) away from patient (A) and placed the patients in separate rooms. A registered nurse (RN) assessed patient (A), noting a four centimeter laceration to the inside of her lower lip. The area was cleaned and patient (A) was given an ice pack and pain medication. Her vital signs were within normal limits. Patient (A) denied fearing patient (B). The physician was notified and orders were written to place patient (B) on assault precautions and peer restriction. Patient (B) was uninjured. The clinical treatment team was evaluating a possible transfer of patient (B) to another unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
