DESCRIPTION OF OCCURRENCE:
On 04/25/16 male patient (B), in his 20s, punched another male patient (A), who was in his 40s.
The facility conducted an internal investigation and notified the police. It was reported patient (A) was in the day hall speaking under his breath when patient (B) walked up to patient (A) and punched him in the face at least three times with a closed fist. Staff placed patient (B) in a hold and placed him in flex cuffs as he was taken back to a locked seclusion room. Nursing assessed patient (A) and he had swelling to his right jaw. The x-ray was negative for a fracture. Pain medications were offered but patient (A) declined. Patient (B) reported patient (A) had been making racial comments to him. Assault precautions and peer restrictions were implemented for the patients. Patient (B) was moved to a different wing on the unit away from patient (A).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
