DESCRIPTION OF OCCURRENCE:
On 04/23/16 a female patient (A), in her 20s, assaulted another female patient (B), who was in her 20s.
The facility conducted an internal investigation and notified the physician and police. Staff observed patient (A) standing behind patient (B) with both of her hands on patient (B)'s neck. Patient (A) then stepped back from patient (B) with her hands in the air and stated patient (B) should not have called her a racially inappropriate name. Staff intervened and separated the patients. Nursing assessed patient (B), who reported she had neck pain. Medications and an ice pack were offered to assist with pain management and patient (B) reported she was fearful of patient (A). Staff asked patient (A) to take a time out in her room. Patient (B) stated patient (A) approached her from behind and pushed her into a fence which caused her neck to hurt. Staff reported patient (A) did not push patient (B). Patient (A)'s medications were adjusted and a peer restriction along with assault II precautions were also implemented for patient safety. Staff reported no further incidents have occurred between the two patients.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
