DESCRIPTION OF OCCURRENCE:
On 04/17/16 male patient (B), in his 50s, hit male patient (A) in the mouth. Patient (A) was in his 20s.
The facility conducted an internal investigation and notified police. Patient (B) came out of his room, yelling in an aggressive manner. He approached patient (A) and hit him in the mouth. Patient (A) walked away from patient (B). Patient (B) continued to follow him and staff intervened and separated them. Patient (A) was assessed and had a cut to his lower lip. Patient (A) rinsed his mouth and no other treatment was necessary. Patient (B) was given a prn (as needed) medication for agitation. He was assessed and had an abrasion on his left hand which was cleaned and bandaged. His left wrist was swollen and an x-ray was taken which was negative for a fracture. He was medicated for pain. Patient (B) was moved to another wing and placed on assault precautions, a peer restriction from patient (A) and wing observation.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
