DESCRIPTION OF OCCURRENCE:
On 03/22/16 male patient (A), in his 40s, pushed male patient (B), who was in his 50s.
The facility conducted an internal investigation and notified the police department. Staff heard loud noises in the therapy room and upon entering, patient (B) stated patient (A) shoved him. Patient (A) stated patient (B) made derogatory comments while walking toward him so he pushed patient (B). Both patients had mental health diagnoses and each had a history of behavior problems which had been care planned. Patient (B) was assessed and noted to have a 1/2 inch abrasion on his left arm from bumping the wall when he was pushed. Patient (A) and patient (B) participated in Mediation which was successful. Patient (A) was also counseled on inappropriate behavior and possible consequences including discharge from the program. The two patients were housed in different locations in the facility.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
