DESCRIPTION OF OCCURRENCE:
On 02/19/16 a male patient, in his 30's, signed out of the facility to go to a laboratory. The patient never showed up for his laboratory appointment and failed to return to the facility until approximately seven or eight hours later.
The facility conducted an internal investigation and notified the police and physician. The patient was admitted to the facility for treatment of multi-substance abuse and schizoaffective disorder as a condition of probation for criminal trespassing. He was noted to be at lunch at 12:30 p.m. on 02/19/16, but could not be found by 2:30 p.m. The hospital police were notified and a search was conducted without locating the patient. The patient was not considered a danger to himself or others, but a risk factor for substance use was present. He returned to the facility about seven hours later and asked to be able to return to the program. However, he violated program rules while he was out and was not permitted to return. The patient was informed he could re-apply for the program when he was capable of following program rules. The patient was picked up by a family member and discharged from the program.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
