DESCRIPTION OF OCCURRENCE:
On 02/18/16 male patient (A), in his 60s, punched male patient (B), who was in his 50s.
The facility conducted an internal investigation and notified police. Both patients were in the day hall. Patient (A) got close to patient (B) who yelled, "Hey watch out." Patient (A) responded by punching patient (B) in the left eye with a closed fist. Staff immediately separated the patients. Nursing assessed patient (B), who had redness of his left eye and his eye was watering. Patient (B) complained of pain and first aid treatment was provided. Staff reported patient (B) did not appear fearful and he did have a diagnosis of dementia with limited memory recall. Staff reported patient (A)'s story changed several times from patient (B) had raised his hand towards him so he struck out at patient (B) and hit him first. Patient (A) was placed on assault II precautions and staff will continue to monitor and keep the patients separated.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
