|Facility: COLORADO MENTAL HEALTH INSTITUTE AT PUEBLO-PSYCH
Date of Occurrence: 2/10/2016
Report Timely: Yes
Type of Occurrence: Abuse/Physical
DESCRIPTION OF OCCURRENCE:
On 02/10/16 two male patients engaged in a physical altercation. Patient (A) was in his 60s and patient (B) was in his 70s.
The facility conducted an internal investigation and notified the physician and hospital police. These two patients started arguing and began swinging at each other. Staff intervened immediately and were able to separate the two patients. Patient (B) sustained an abrasion to his cheek and patient (A) sustained some scratches. Both patients were provided with first aid treatment. Neither patient reported having any pain and staff reported the patients did not appear to be fearful. Both patients had diagnoses of dementia with behaviors. Staff believed patient (A) became angry because patient (B) had gone into his room when he was not present. Assault II precautions were implemented for patient (A) and staff will continue to monitor both patients.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
