DESCRIPTION OF OCCURRENCE:
On 01/23/16 male patient (A), in his 30s, struck male patient (B), who was in his 50s.
The facility conducted an internal investigation and notified the police department. Patient (A) and patient (B) were sitting next to each other on the couch. Patient (A) walked away and started back to the couch with a paper. Patient (A) dropped the paper and patient (B) picked the paper up and handed it back to patient (A). Patient (A) began hitting patient (B). Staff separated the patients. Both patients had mental health diagnoses and histories of behavior problems which had been care planned. Patient (A) was placed in locked door seclusion and was placed on assault II precautions. Patient (B) was assessed and noted to have a bruise on his left ear. The patients were housed on separate wings of the unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
