DESCRIPTION OF OCCURRENCE:
On 01/17/16 male patient (A), in his 20s, struck male patient (B) with a board. Patient (B) was in his 30s.
The facility conducted an internal investigation and notified the police department and the physician. Patient (A) had broken a board off of a wardrobe unit and attacked patient (B). Patient (A) struck patient (B) multiple times and patient (B) attempted to block the blows using his arms. Staff intervened and separated the patients. Patient (A) was escorted to the seclusion room and placed in restraints. Patient (B) had small abrasions on his left arm. X-rays of the patient's arm were taken and the results were negative for a fracture. Both patients had histories of behavior problems which had been care planned. Patient (B) was interviewed and denied there had been any problems or conflicts with patient (A) before the assault occurred. Patient (A) was moved back to the unit he came from and away from patient (B). Patient (A's) psychiatrist was looking into court ordered medications once again. Patient (B) decided to press charges against patient (A).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
