DESCRIPTION OF OCCURRENCE:
On 01/15/16 male patient (A), in his 20s, physically assaulted male patient (B), who was also in his 20s.
The facility conducted an internal investigation and notified the physician. As patients were exiting their rooms, patient (A) approached patient (B) from behind and placed patient (B) in a choke hold. Patient (A) proceeded to hit patient (B) and both struggled with each other. Both patients then fell to the floor. Staff intervened and separated them. Patient (A) was escorted to the seclusion room. Nursing assessed patient (B) and he had complaints of finger pain and redness to his neck. Ice was applied and medications administered for pain. Staff reported patient (A) has been on assault II precautions and suicide precautions with medication adjustments. Patient (B) stated he was not fearful of patient (A) and had no idea why he was targeted. Patient (B) moved to a different wing and staff was instructed to continue patient (A)'s current plan of care.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
