DESCRIPTION OF OCCURRENCE:
On 01/11/16 male patient (A), in his teens, assaulted male patient (B), who was also in his teens.
The facility conducted an internal investigation and notified administrative personnel and hospital security. During group, patient (A) approached patient (B) and started hitting him. Patient (B) was attempting to protect himself by covering his head and then began hitting back. Staff was able to remove patient (B) from the area as patient (A) remained aggressive, throwing items in the room. Staff contained patient (A) and placed him in seclusion. Three staff members were injured while attempting to manage patient (A). Nursing assessed patient (B) and he had swelling, bruising and an abrasion to his head. X-rays were obtained which showed no evidence of a fracture. Staff offered an ice pack for the swelling and pain medications for patient (B). Patient (B) reported he was fearful of patient (A). Assault II precautions and peer restrictions were implemented for patient (A). The two patients were housed on separate floors and staff will continue to monitor them. Also, staff was attempting to facilitate some type of mediation between the two patients.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
