DESCRIPTION OF OCCURRENCE:
On 12/30/15 a female patient, in her 30s, eloped.
The facility conducted an internal investigation and notified the police. Staff reported the patient had been angry earlier in the day and was threatening to leave. Staff provided counseling and the patient had agreed to remain in treatment. Around 10:30 p.m., staff was unable to locate the patient in her bedroom. The patient belongings remained in her room. A search was conducted and the patient was not found. The patient was her own responsible party and was not considered to be at risk. The patient had a diagnosis of mental illness and substance abuse. The patient did not return to the program.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
