DESCRIPTION OF OCCURRENCE:
On 12/24/15 male patient (A), in his 50s, hit male patient (B), who was in his 20s.
The facility conducted an internal investigation and notified hospital security and administrative personnel. As the patients were standing in the medication line, patient (A) started hitting patient (B). Patient (B) had wrist restraints. One staff member got between the two patients and was struck several times by patient (A) before other staff was able to remove patient (A) to a seclusion room. Patient (B) sustained a swollen and bruised lip and first aid was provided. Patient (B) denied being fearful of patient (A). Assault II precautions with a peer restriction were implemented for patient (A). Staff moved patient (B) to another unit to further separate him from patient (A).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
