DESCRIPTION OF OCCURRENCE:
On 12/13/15 male patient (A), in his 20s, assaulted male patient (B), who was in his 30s.
The facility conducted an internal investigation and notified the physician and security. Staff observed patient (A) on top of patient (B) with his arms swinging at him. Staff immediately intervened, restrained patient (A) and removed him to the seclusion room. Nursing assessed patient (B) and he sustained a bloody nose with several abrasions. First aid treatment was provided. Patient (B) reported patient (A) approached him with a metal object and started hitting him for no reason. Staff reported patient (B) did not say he was fearful of patient (A). Staff found a piece of metal missing from a box located in the bathroom and believed patient (A) was trying to use this piece of metal to physically attack patient (B). Assault II precautions were implemented for patient (A) and he was moved to a different wing.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
