DESCRIPTION OF OCCURRENCE:
On 12/02/15 a male patient, in his 30s, eloped from the community reintegration program.
The facility conducted an internal investigation and notified the physician and hospital security. Around 9:50 a.m., staff received a phone call from the police inquiring about this male patient as he was currently at the local jail. The patient was supposed to be on an authorized outing and On-Ground Unsupervised Privileges. Review of documentation showed the patient signed out at 8:30 a.m. with an expected return of 10:30 a.m. The patient allegedly left the premises and went to the police station to report he felt he was being abused. The patient had a diagnosis of mental illness with a history of elopement. Staff reported the patient was responsible for himself and could be at risk if he did not take his medications. The patient's stay was involuntary. The facility investigation determined the patient made up the allegations and he later denied any type of abuse was occurring. Upon the patient's return, he was placed on a locked unit with a higher level of security.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation and placing the patient on a locked unit.
The Department will review this occurrence prior to the next survey.
