DESCRIPTION OF OCCURRENCE:
On 11/16/15 male patient (A), in his 20s, hit male patient (B), who was in his 50s.
The facility conducted an internal investigation. The facility notified the police department. The two residents were playing chess at a table in the day hall. They were talking about the rules of the game when patient (A) stood up and hit patient (B) in the left eye. Staff intervened and separated the patients. Both patients had mental health diagnoses and a history of behavior problems. Staff escorted patient (A) to seclusion and placed him in restraint. Patient (B) was assessed and noted to have swelling of his left eye area. Ice was applied every 8 hours. Patient (A) was placed on assault II precautions with two-to-one staff monitoring. Patient (B) was moved to a different wing on the unit, away from patient (A).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, separating the patients, placing patient (A) in seclusion with restraint, assessing and treating patient (B), placing patient (A) on assault II precautions with two-to-one staff monitoring, and moving patient (B) to a different wing on the unit.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
