DESCRIPTION OF OCCURRENCE:
On 11/11/15 male patient (A), in his 20s, hit male patient (B), also in his 20s.
The facility conducted an internal investigation. The facility notified the police department. Patient (A) punched patient (B) in the face. Staff intervened and instructed patient (A) to go to his room. Patient (B) was assessed and was noted to have a laceration above his right eye. He was transferred to the hospital for further evaluation and treatment. When he returned he was placed on assault II precautions and peer restriction from patient (B). Both patients had mental health diagnoses with histories of behavior problems. Patient (A) was started on emergency medications.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, separating the patients, assessing patient (B), transferring patient (B) to the hospital for further evaluation and treatment, placing patient (B) on assault I precautions and peer restriction, placing patient (A) on assault II precautions and peer restriction, and starting patient (A) on emergency medications.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
