DESCRIPTION OF OCCURRENCE:
On 11/8/15 male patient (C) hit male patient (B). Both patients were in their teens.
The facility conducted an internal investigation. The facility notified the physician and hospital police. Three patients were attempting to engage in a physical altercation. Patient (A) and patient (B) were lunging towards each other with clenched fists. Staff intervened and placed patient (B) in a manual hold. At that point, patient (C) pushed the staff member and hit patient (B)'s face. Staff separated the patients. Nursing assessed patient (B) who complained of left temple and nose pain. Tylenol was administered. There was no redness or swelling. His neurological status was monitored.. Patient (C) was escorted to his room to calm down. The patients' care plans were updated to reflect this altercation. None of the patients stated they were fearful. Assault precautions were implemented for patient (C).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, assessing and treating patient (B), revising the patients' care plans and implementing assault precautions for patient (C).
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
