DESCRIPTION OF OCCURRENCE:
On 11/1/15 male patient (A) hit male patient (B) in the neck. Both patients were in their 20s.
The facility conducted an internal investigation. The facility notified the police, physician, and administrative personnel. Patient (A) was walking behind patient (B) and hit him in the neck. Patient (B) was listening to music with headphones and did not see the assault coming. As staff approached, patient (A) retreated to his room and locked the door. Patient (B) sustained swelling of the neck and complained of pain. Tylenol was administered, ice applied and an x-ray was ordered. He declined to have the x-ray. He denied being fearful of patient (A). Patient (A) stated patient (B) had talked about him and called him a derogatory name. Patient (A) was undergoing medication changes for psychosis. He was moved to a different unit and placed on assault II precautions.
The Department reviewed the facility's report and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, assessing the patients, moving patient (A), implementing assault II precautions and conducting an internal investigation.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
