DESCRIPTION OF OCCURRENCE:
On 10/29/15 two male patients engaged in a physical altercation. Patient (A) was in his 20s and patient (B) was in his 20s.
The facility conducted an internal investigation. The facility notified the police, physician and administrative personnel. Patient (A) struck patient (B) in the face. Patient (B) responded by hitting patient (A) which knocked him to the floor. Patient (B) continued to fight and he stomped on patient (A)'s arm. Patient (A) reported he was kicked in the head several times. Staff intervened and separated the patients. Both patients sustained injuries and complained of pain. Patient (A) was transferred to the hospital for further assessment. He returned and was diagnosed with a closed head injury with a head contusion, cervical strain and right arm contusion. No additional treatments were necessary. Neither patient reported they were fearful. Staff reported these patients were already on assault II precautions when this altercation occurred. Patient (A) had been given medications about 2.5 hour prior to this assault due to another incident. His medications were going to be reviewed again for any needed adjustments to address his psychosis. Patient (B) was moved to a different unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, assessing the patients, transferring patient (A) to the hospital for further assessments, reviewing medications, continuing assault II precautions and moving patient (B) to another unit.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
