DESCRIPTION OF OCCURRENCE:
On 10/23/15 male patient (A), in his 40s, struck male patient (B), who was also in his 40s.
The facility conducted an internal investigation. The facility notified the police department. Staff observed patient (A) punch patient (B) on the nose in the hallway. Staff contained patient (A) and placed him in locked seclusion. Patient (B) was assessed and transferred to the hospital where he was treated for a laceration on the left side of the bridge of his nose and a nasal fracture. Both patients had mental health diagnoses and a history of behavior problems. Patient (A) admitted to striking patient (B) because patient (B) brushed up against him and got in his face. Patient (A) was removed from locked seclusion and placed on assault II precautions and moved to a higher security level unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, removing patient (A) from the area and placing him in locked seclusion, assessing patient (B) and transferring him to the hospital for further evaluation and treatment, placing patient (A) on assault II precautions, and moving patient (A) to a higher security level unit.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
