DESCRIPTION OF OCCURRENCE:
On 10/09/15 female patient (A), in her teens, struck female patient (B), who was also in her teens.
The facility conducted an internal investigation. The facility notified the physician, families/guardians and the police department. The two patients were getting ready to attend a group meeting when they began to argue. The situation escalated and patient (A) jumped up from the couch and struck patient (B) on the face. Staff intervened and separated the patients. Patient (A) was placed in seclusion and restraint. Both patients had mental health diagnoses and a history of behavior problems which had been care planned. Patient (B) was assessed and noted to have superficial lacerations to the inside of her lip. Patient (B) stated she was not fearful of patient (A) and stated they knew each other from a previous placement. Patient (A's) physician wrote orders to place her on assault II precautions.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, separating the patients, placing patient (A) in seclusion and restraint, assessing and treating patient (B), interviewing patients and staff, and obtaining orders to place patient (A) on assault II precautions.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
