DESCRIPTION OF OCCURRENCE:
On 10/10/15 male patient (A), in his 20s, and male patient (B), in his 40s, were involved in a physical altercation.

FACILITY ACTION:
The facility conducted an internal investigation. The facility notified the police department. Patient (B) was in the day hall watching television. Patient (A) began yelling, swearing, and then struck patient (B) with a closed fist in the mouth, nose, and forehead. Patient (B) began swinging at patient (A) in self-defense and struck patient (A) in the right eye. Patient (A) was contained and escorted to locked door seclusion. Both patients had mental health diagnoses and a history of behavior problems which had been care planned. Both patients were assessed and patient (B) was noted to have a small abrasion to the inside of his upper lip, bruising to his forehead, a small laceration on his left hand and his nose was bleeding. Patient (A) sustained a small abrasion to the inside of his upper lip and redness to his right eye. The injuries were cleaned and treated. Patient (A) was placed on assault II precautions and on peer restriction from patient (B). The following day, patient (A) was placed in a higher security level unit.

DEPARTMENT FINDINGS:
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, separating the patients, placing patient (A) in locked door seclusion, assessing and treating both patients, interviewing patients and staff, placing patient (A) on assault II precautions and on peer restriction from patient (B), and placing patient (A) in a higher security level unit.

The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
