DESCRIPTION OF OCCURRENCE:
On 09/28/15 two male patients were involved in a physical altercation. Patient (A) was in his 40s and patient (B) was in his 30s.
The facility conducted an internal investigation. The facility notified hospital police and administrative personnel. Patient (A) ran towards patient (B), who was talking on the phone. Patient (B) reported patient (A) hit him slightly on the shoulder and he also hit his wrist on the wall. Staff intervened and separated the patents. Nursing assessed patient (B) and noted his wrist was swollen. Ice was applied and no further treatment was necessary. Assault 2 precautions were implemented for patient (A) and a peer restriction was started for both patients. When patient (A) was informed of the assault 2 precautions, he began punching the walls and sustained abrasions to both hands and an x-ray was ordered and was negative for any fractures. Patient (B) denied being fearful but was "cautious" of patient (A). The patients were housed on separate wings of the unit.
The Department reviewed the facility's report and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, assessing the patients, providing first aid to patient (B), assessing patient (A), implementing assault precautions for patient (A) and placing both patients on peer restriction.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
