DESCRIPTION OF OCCURRENCE:
On 09/24/15 it was alleged that male patient (C), in his 40s, exposed himself to female patient (B), who was in her 30s.

FACILITY ACTION:
The facility conducted an internal investigation. The facility notified the police and administrative personnel. Staff heard patient (A) yell, "You pervert" as she was walking towards patients (B) and (C). Patient (A) alleged patient (C) showed patient (B) his penis. Staff intervened and separated the patients. Patient (C) stated patient (B) asked to see his penis so he showed her. Patient (B) denied this. Patient (B) stated she was not fearful or upset. Peer restrictions from all females were initiated for patient (C) and he was not allowed to go outside for breaks. Patient (B) was placed on peer restriction from patient (C).

DEPARTMENT FINDINGS:
The Department reviewed the facility's report and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, conducting an internal investigation and placing patient (C) on peer restrictions.

The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
