DESCRIPTION OF OCCURRENCE:
On 07/08/15 female patient (A), in her 40s, reported a staff member spoke in a derogatory manner to female patient (B).
The facility conducted an internal investigation. The facility notified the police department. Patient (A) requested to speak with the psychologist after group. Patient (A) stated she heard patient (B) call a staff member a punk. Patient (A) stated she then heard the staff member use foul language towards patient (B). Patient (B) was interviewed and verified the information. Both patients reported feeling afraid of the staff member. The staff member was suspended during the investigation. He denied the allegation. The staff member had been involved in a previous abuse allegation and received a corrective action related to that incident. A disciplinary hearing was held and the staff member was issued a Performance Improvement Plan. The staff member will attend training, participate in structured supervision, and was transferred to another unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, interviewing patients and staff, suspending a staff member during the investigation, holding a disciplinary hearing, issuing a Performance Improvement Plan, providing training and supervision, and transferring the staff member to another unit.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
