DESCRIPTION OF OCCURRENCE:
On 06/26/15 male patient (A), in his 20s, struck male patient (B), who was in his 50s.
The facility conducted an internal investigation. The facility notified the police department. Patient (A) was informing staff about voices he had been hearing. Patient (A) then ran toward the quiet living area. Staff ran after patient (A) and observed him standing over patient (B). Patient (B) reported he had been struck once in the face and had fallen backward striking his head on the floor. Patient (B) was assessed and noted to have sustained a laceration on the bridge of his nose and another laceration on the left side of his head. Patient (B) was transported to the hospital for further evaluation and treatment. Both patients had been diagnosed with mental health issues and had histories of behavior problems. Resident (B) was moved to another unit away from patient (A). Resident (A) had been started on emergency medication and was placed on assault ll precautions.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, interviewing patients and staff, assessing patient (B), transferring patient (B) to the hospital for further evaluation and treatment, moving patient (B) to a different unit, starting patient (A) on emergency medications, and placing patient (A) on assault ll precautions.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
