DESCRIPTION OF OCCURRENCE:
On 06/19/15 female patient (B), in her 30s, was physically assaulted by female patient (A) who was in her 40s.
The facility conducted an internal investigation. The facility notified the physician, police department, and appropriate facility administration and personnel. Patient (B) was sitting on the couch when patient (A) walked up behind her, grabbed her hair and pulled it. The patients were separated and patient (A) was escorted to the seclusion room. Patient (B) was assessed and there were no injuries noted. Patient (B) filed charges against patient (A). Staff interviews revealed patient (A) grabbed patient (B)'s hair without provocation. The facility placed patient (A) on assault II precautions, including but not limited to increased supervision and frequent checks.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, escorting patient (A) to seclusion, assessing patient (B), conducting an investigation, and initiating assault precautions on patient (A).
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
