DESCRIPTION OF OCCURRENCE:
On 06/19/15 the facility determined 6 tablets of Lovaza, prescibed for a male patient, were missing. The patient was in his 70s.
The facility conducted an internal investigation. The facility notified police and appropriate facllity personnel. The medication was discovered missing when staff were setting up morning medications. A search was conducted but the Lovaza tablets were not located. The patient's aspirin 81 mg. was also missing and later found in a Hippa bag. This was a bag containing empty bottles to be returned to the pharmacy. The facility was unable to determine what happened to the medication or to identify a suspect. The hospital installed video camera equipment in the medication room.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting a search and investigation and installing video camera equipment in the medication room.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
