DESCRIPTION OF OCCURRENCE:
On 06/16/15 a male patient, in his 60s, reported his wallet was missing.

FACILITY ACTION:
The facility conducted an internal investigation. The facility notified police and the physician. The patient said his brown leather wallet was missing. He reported it did not contain any money but had a family photo in it. His personal clothing sheet was checked and did not list a wallet. Staff reported they had never seen a wallet. A search was conducted but it was not located. The facility was not able to substantiate that the resident had the reported item.

DEPARTMENT FINDINGS:
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting a search and investigation.

The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
