DESCRIPTION OF OCCURRENCE:
On 06/10/15 male patient (B), in his 20s, punched male patient (A), who was in his 20s.
The facility conducted an internal investigation. The facility notified administrative personnel and internal security. Staff reported patient (A) walked by patient (B) and asked if he was all right. Patient (B) responded by punching patient (A) on the neck. Patient (B) then walked to the seclusion room for a voluntary time out. Staff assessed patient (A) for injuries. He had redness and swelling to his neck with complaints of pain. Neurological checks and vital signs were within normal limits. An ice pack was applied and he was medicated for pain. These two patients had a peer restriction in place due to a previous altercation and patient (A) had been re-directed several times to stay away from patient (B). Patient (A) reported he has been compelled to go near patient (B). Assault II precautions were already in place for patient (B) and he was moved to another unit. Both patients have a behavioral and mental health history. Administration gave patient (A) an infraction for violating the peer restriction, reviewed his medications and implemented assault I precautions. Patient (A) reported he was fearful of patient (B) but felt compelled to be near him despite the peer restriction in place.
The Department reviewed the facility's report and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an internal investigation, assessing the residents, providing treatment, moving patient (B) to another unit, implementing assault precautions I on patient (A).
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
