DESCRIPTION OF OCCURRENCE:
On 06/05/15 male patient (A), in his 60s, hit male patient (B), who was in his 50s.
The facility conducted an internal investigation. The facility notified the physician, appropriate administrative personnel and internal security. Staff reported patient (A) was yelling out and as staff attempted to redirect him, he began to charge a staff member. Patient (B) responded by yelling at patient (A), who reacted by hitting patient (B) on the cheek. Staff separated the patients and assessed patient (B) for injuries. Patient (B) sustained a bruise to his cheek and reported pain. Pain medication and ice were administered. Assault II precautions were implemented for patient (A) and a medication review occurred. Staff reported patient (B) had a diagnosis of dementia and did not recall the event or exhibit any signs of being fearful. Staff will continue to monitor, intervene as necessary and keep the patients separated.
The Department reviewed the facility's report and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, assessing the patients, treating patient (B), reviewing the care plans and monitoring the patient's behaviors.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
