DESCRIPTION OF OCCURRENCE:
On 05/27/15 female patient (B), in her 60s, was physically assaulted by female patient (A), who was in her 60s.
The facility conducted an internal investigation. The facility notified the police department and appropriate facility administration and personnel. Patient (B) reported patient (A) had attempted to punch her and patient (B) scratched patient (A) in self-defense. Patient (B) was assessed and there were no injuries noted. Patient (A) was assessed and had several bruises and scratches which were treated. Patient (A) was given medication for agitation. There were no witnesses to the incident. Documentation review revealed patient (A) had been having increased psychiatric symptoms. Patient (A) was placed on 1:1 supervision and moved to a room closer to the nurse's station to allow closer monitoring.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, assessing both patients, providing treatment for patient (A), conducting an investigation, initiating 1:1 supervision for patient (A), and moving patient (A)'s room closer to the nurse's station.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
