Date of Occurrence: 5/18/2015
Report Timely: Yes
Type of Occurrence: Abuse/Physical
DESCRIPTION OF OCCURRENCE:
On 05/18/15 female patient (A) was struck by female patient (B). Patient (A) was in her 30s, and patient (B) was in her 20s.
The facility conducted an internal investigation. The facility notified the police and appropriate administrative personnel. Both patients were in the hallway when patient (A) yelled for help indicating that patient (B) had just punched her in the nose. Staff responded and found patient (A) with a nose bleed and complaining of pain to the area where she had just been struck. Resident (B) was wheeling herself away in her wheelchair. Patient (B) was immediately placed on Assault II Precautions, and both patients were placed on peer restriction. Patient (A) was assessed and there was no evidence of fracture or dislocation. Over-the-counter medication was administered and she declined an offer of ice to apply to the area. She was interviewed and stated that patient (B) struck her when she asked patient (B) to quit yelling. When questioned, patient (B) stated that she was anxious and upset and reacted aggressively when patient (A) asked her to lower her voice. She stated that she should not have hit her. Patient (A) stated she was not fearful because they were on peer restriction, and that she was faithfully keeping herself away from patient (B).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, placing patient (B) on Assault II Precautions, assessing and treating patient (A), and placing the patients on peer restriction.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
