DESCRIPTION OF OCCURRENCE:
On 05/18/15 patient (A) was punched in the face by patient (B). The patients were both male. Patient (A) was in his 20, and patient (B) was in his 30s.
The facility conducted an internal investigation. The facility notified the police and the physician. Patient (B) approached patient (A) and began punching him in the face with a closed fist. The incident was unprovoked. Patient (B) was contained by staff and placed in locked door seclusion. Patient (A) was assessed and had sustained a cut on the bridge of his nose, a possible nasal fracture, and had complaints of pain in his upper jaw. Tylenol was administered and an ice pack applied to his jaw and nose. X-rays were ordered. Patient (A) was moved of the unit away from patient (B). Patient (B) had been increasingly psychotic and unpredictable. He was placed on assault II precautions and had changes to his medications. Staff was aware of his behaviors and were monitoring his activities more closely when he was out of his room or in the day hall. At the time of this report patient (B) had been improving and was less agitated.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, removing patient (B), assessing and treating patient (A), moving patient (A) to a different unit, placing patient (B) on assault II precautions, and modifying his medications.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
