DESCRIPTION OF OCCURRENCE:
On 05/16/15 staff reported 9 capsules of Vistaril were missing. The medication had been prescribed for a male patient, in his 60s.
The facility conducted an internal investigation. The facility notified police and facility personnel. A search was conducted but the missing medication was not located. Medication administration staff were interviewed. One staff member stated she thought she had left the medication cart unlocked when she went to take medication to several patients. The cart was in the locked medication room when this occurred. No suspect was identified. The facility will install cameras in the medication room on this unit. A corrective action was taken with the staff member who left the medication cart unlocked.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting a search and investigation, installing cameras in the medication room and taking a corrective action with staff.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
