DESCRIPTION OF OCCURRENCE:
On 04/20/15 a male patient, in his 30s, eloped.

FACILITY ACTION:
The facility conducted an internal investigation. The facility notified physician and police. The patient was in a substance abuse treatment program. He was not considered to be at risk and and was responsible for himself. He was determined to be missing after a 9:30 p.m. wellness check. A search was conducted but he was not located. The patient had a history of elopement and will not be re-admitted to the program until the issue of elopement is addressed.

DEPARTMENT FINDINGS:
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting a search and investigation.

The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
