DESCRIPTION OF OCCURRENCE:
On 04/16/15 a female patient (B) in her 60s was physically assaulted by a male patient (A) in his 60's.

FACILITY ACTION:
The facility conducted an internal investigation. The facility notified the physician, police department, and appropriate facility administration and personnel. Both patient (A) and patient (B) have mental health diagnoses. Staff members heard patient (B) yelling for help when they witnessed patient (A) hitting patient (B) with a closed fist multiple times as they ran to separate the patients. Staff members placed patient (A) in a manual hold until patient (A) calmed down. Patient (B) was taken to her room and assessed. Patient (B) sustained multiple hematomas and a couple skin tears to her upper extremities. Patient (B) was provided treatment and neurological assessments were initiated. Patient (B) admitted she had struck patient (A) first. Patient (A) was assessed and sustained no injuries. Staff member interviews corroborated the incident and revealed there were no previous incidents between patient (A) and patient (B). Documentation review confirmed the incident. The allegations were substantiated. The facility has modified patient (A)'s plan of care to reflect assault precautions and moved patient (A) to a locked, high security unit.

DEPARTMENT FINDINGS:
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, assessing both patients, providing treatment to patient (B), conducting an investigation, implementing assault precautions for patient (A), and moving patient (A) to a locked, high security unit.

The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
