DESCRIPTION OF OCCURRENCE:
On 04/04/15 a male patient (B) in his 70s was physically assaulted by a male patient (A) in his 20s.

FACILITY ACTION:
The facility conducted an internal investigation. The facility notified the physician, police department, and appropriate facility administration and personnel. Both patients have psychological health diagnoses. Patient (B) sat down on the couch next to patient (A) and proceeded to throw crackers at patient (A) when patient (A) retaliated by punching patient (B) in the head. Staff members intervened and separated the patients. Patient (B) was assessed having sustained a small laceration to his head and treatment was provided. Patient (B) did not express fear of patient (A). Both patients agreed to stay away from one another. Patient (A) was placed on assault precautions to include increased observation and restrictions for off-unit activities. The facility has modified patient (A)'s plan of care to include assault precautions.

DEPARTMENT FINDINGS:
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, assessing and providing treatment to patient (B), conducting an investigation, having both patients agree to stay away from one another, and implementing assaults precautions for patient (A).

The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
