DESCRIPTION OF OCCURRENCE:
On 03/14/15 female patient (A), in her 30s, and female patient (B), in her 20s, made threatening remarks to each other.

FACILITY ACTION:
The facility conducted an internal investigation. The facility notified the physician and the police department. Patient (A) was making menacing and threatening remarks to patient (B). Patient (B) then made a threatening remark back to patient (A). Staff notified the facility police and they interviewed both patients. Neither patient wanted to file charges and both patients agreed to stay away from each other. Patient (B) stated she was fearful of patient (A). Both patients had mental health diagnoses and a history of behavior problems which had been care planned. The patients were placed on peer restriction and they did not have any further problems following the incident.

DEPARTMENT FINDINGS:
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, interviewing patients and staff and placing the patients on peer restriction.

The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
