DESCRIPTION OF OCCURRENCE:
On 2/24/15 a male patient, in his 60s, reported a phone card was missing.
The facility conducted an internal investigation. The physician and campus police were notified. The patient received a phone card valued at $30.00 in the mail from a family member. Staff encouraged him to put the card in the safe and he was reminded of the policy for safe keeping of items. He refused and kept the card in his wallet. On 22/24/15, the card could not be found. A search was conducted but the phone card was not located. The facility noted there had been a pattern of several lost phone cards and money. Staff were interviewed but could provide no information about the missing phone card. There was no perpetrator identified. Staff re-educated the patient on policies and using the safe.
The Department reviewed the facility's report and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an internal investigation, conducting a search, interviewing staff and re-educating the patient on policies for safe keeping of materials.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
