DESCRIPTION OF OCCURRENCE:
On 02/14/15, two male patients were involved in a physical altercation. Both patients were in their 20s.
The facility conducted an internal investigation. The physician and police were notified. Staff witnessed patient (A) standing over patient (B) hitting him in the face several times. Staff immediately intervened and separated them. Staff assessed patient (B) and he had redness and swelling to the left eye and his upper lip was swollen with a small abrasion. Treatment was provided. He complained of back pain and medications were administered for pain. Patient (A) was moved to another unit away from patient (B). Patient (B) stated the attack was unprovoked. No charges were pressed. Patient (A) had a behavioral care plan in place and it was reviewed and revised. Assault II precautions and medications were implemented for safety. The facility concluded the allegation was substantiated.
The Department reviewed the facility's report and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, assessing patient (B), treating patient (B), removing patient (A) from the area, conducting an internal investigation, implementing assault II precautions and reviewing the medications and care plan.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
