DESCRIPTION OF OCCURRENCE:
On 01/31/15 patient (A) hit patient (B) in the eye and attempted to choke him. Both patients were males in their 50s.
The facility conducted an investigation and notified police and the physician. Patient (A) was in the shower. Patient (B) knocked on the door to see if it was occupied)
Patient (A) began yelling and threatening patient (B). Patient (A) exited the shower room and attacked patient (B).He punched him in the left eye and tried to choke him. Patient (B) began to defend himself and hit patient (A). Staff separated them. Patient (A) then picked up a chair and threw it at the window. He as taken to the seclusion room. Both patients were assessed. Patient (A) had scratches on his knees that were cleaned and a Band-aid applied. Patient (B) had redness to his neck and left eye. He did complain of pain but refused an ice pack and pain medications. The patients were placed on peer restriction. Patient (A) was placed on assault precautions and the physician reviewed his current medication regime. Patient (A) was transferred to another unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons, assessing both patients, treating patient (A), taking patient (A) to seclusion and subsequently placing him on assault precautions. placing both patients on peer restriction, evaluating patient (A)'s medications and moving patient (A) to another unit.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
