DESCRIPTION OF OCCURRENCE:
On 01/23/15 female patient (A), in her 20s, physically assaulted female patient (B), who was in her teens.
The facility conducted an internal investigation. The facility notified the physician and the police department. The two patients were in the day hall and began to argue. Patient (A) attempted to hit patient (B) but staff intervened. Patient (A) was able to grab patient (B's) hair causing her to cry. Staff escorted patient (A) to the seclusion room. Patient (B) was escorted to the quiet living area and assessed. There was no visible injury to her scalp, but she sustained 2 minor scratches on her left arm. Both patients had a mental health diagnoses and a history of behavior problems which had been care planned. Patient (A) was placed on assault ll precautions.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, separating the patients, placing patient (A) in locked door seclusion, interviewing patients and staff, assessing patient (B), and placing patient (A) on assault ll precautions.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
