DESCRIPTION OF OCCURRENCE:
On 01/22/15 male patient (A), in his 20s, and male patient (B), in his 40s, were involved in a physical altercation.
The facility conducted an internal investigation. The facility notified the police department. The patients were in line to receive their medication. Staff heard arguing and saw patient ((A) hit patient (B) on the left cheek. Patient (B) started chasing patient (A) while verbally threatening him. Staff intervened and kept the patients from further physical contact. Patient (B) remained agitated and was escorted to locked door seclusion. He was assessed and noted to have slight swelling and pain to his right cheek. Both patients had mental health diagnoses and a history of behavior problems. They were both placed on assault ll precautions and peer restriction from each other. Patient (A) was moved to a different unit later that day due to ongoing threats from patient (B).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, separating the patients, placing patient (B) in locked door seclusion, assessing and treating patient (B), placing both patients on assault ll precautions and peer restriction from each other, and moving patient (A) to another unit.
The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
