DESCRIPTION OF OCCURRENCE:
On 1/13/15, male patient (A), in his 30s, punched male patient (B), who was in his 50s.

FACILITY ACTION:
The facility conducted an internal investigation. Hospital police were notified and the physician. Patient (B) was standing at the nurses' station mumbling to himself. Patient (A) approached the desk near resident (B), made a derogatory comment to patient (B), and then punched patient (B) in the back of the head. Patient (A) turned around and walked away. Patient (B) leaned forward into the station and then slumped over and lowered himself to the floor. Patient (B) was assessed and had mild swelling to the back of the head. Vital signs were checked and neurological checks initiated. Ice was applied and Tylenol administered for pain. Patient (A) was escorted to the seclusion room, emergency medications administered and assault II precautions implemented. The patient care plans were reviewed. The facility concluded the allegation was substantiated.

DEPARTMENT FINDINGS:
The Department reviewed the facility's report and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, assessing patient (B), providing treatment for patient (B), escorting patient (A) to seclusion, placing patient (A) on assult precautions and administering emergency medications to patient (A).

The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
