DESCRIPTION OF OCCURRENCE:
On 01/04/15 male patient (A), in his 20s struck male patient (B), who was in his 60s.
The facility conducted an internal investigation. The facility notified the police department. Patient (B) walked by patient (A) who was standing next to a trash can. Patient (B) started talking to patient (A) and was close to his face. Patient (A) struck patient (B) on the upper chest causing him to fall to the floor. Patient (A) walked to his room with staff. Patient (B) had a small abrasion to his right elbow from the fall which was cleaned and treated. Both patients were placed on peer restriction from each other. Patient (A) was placed on assault ll precautions. Both patients had a mental health diagnosis and a history of behavior problems. Patient (B) was transferred to another unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, ensuring the patients were separated, assessing and treating patient (B), placing both patients on peer restriction from each other, placing patient (A) on assault ll precautions, and transferring patient (B) to another unit.
The facility is subject to a deficiency for failure to report the occurrence timely. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
