DESCRIPTION OF OCCURRENCE:
On 12/27/14 male patient (A), in his teens, physically assaulted male patient (B), who was also in his teens.
The facility conducted an internal investigation. The facility notified the police department. Patient (B) was leaving the bathroom area and accidentally flicked water on patient (A). Patient (A) pushed patient (B) down and began punching and kicking him. Staff intervened and separated them. Both patients had mental health diagnoses and a history of behavior problems. Patient (A) was escorted to the IET (Intensive Evaluation Treatment) area. He then began hitting the walls and hit a staff member. Both patients were assessed and patient (A) had swelling to his knuckles. Patient (B) was assessed and was noted to have bruising above his right eyebrow and right arm and swelling and bruising of the jaw area. His vital signs and neurological checks were within normal limits. Patient (A) was placed on assault ll precautions. He was housed in the IET area until his behavior was no longer a danger to others.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an internal investigation, separating the patients, assessing the patients, placing patient (A) on assault ll precautions, and housing patient (A) in the IET until his behavior was controlled.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
